Register a death by declaration

How to register a death by declaration at your nearest Register Office if you can't come to a Bristol Register Office.

If someone died in Bristol and you can't come to either of the Bristol Register offices then you must ‘make a declaration’ at your nearest Register Office. 

You’ll need to contact them to make an appointment to ‘make a declaration’.

At your appointment the Registrar will take the details for the death registration and ask you to sign the declaration. 

The paperwork will be sent to Bristol Register Office for the Registrar to enter the registration in the appropriate register. 

Apply for a certificate

After your appointment you can apply for a certificate from Bristol Register Office. 

All certificates are sent by ‘signed for’, make sure you’re able to sign for your certificate at the delivery address.

You can also apply by post or in person at:

Bristol Register Office, The Old Council House, Corn Street, Bristol, BS1 1JG. 

If you apply by post you must include a cheque or postal order for £4 and a stamped self-addressed envelope. Postage for a standard envelope: this will not be sent by signed for delivery.
