Discharge to assess referral form

Use this form if you're a health or social care professional who needs to refer a service user: 

• who's currently getting a service from a Discharge to Assess pathway (Pathway 1, 2 or 3) or 
• who's being supported by Rapid Response to Bristol City Council Discharge to Assess team for an assessment of their care and support needs  

To make the referral you'll need: 

• the service user's consent or a clear reason why you need to refer without it (for example, because they lack capacity to consent or there's a safeguarding concern)
• the service user's basic details 
• the service user's main contacts 
• details of the service user's current situation 
• the reason for referral
