SEND autism support documents for schools

Referral form schools must use to request help from the Bristol Autism Team (BAT), and toolkits to support those with Autistic Spectrum Condition.

If your school has concerns about a pupil and wants to ask for a consultation visit from our team, complete our Bristol Autism Team Referral and Pupil Record form (word doc, 106KB) (opens new window) .

You need to:

  • make sure your pupil meets our criteria on the first page of the form
  • complete the first three sections of the form, so that this can be used as an ongoing pupil record
  • sign the Service Level Agreement
  • get parental signatures, and scan a copy of the page
  • give any evidence of current and reviewed support, such as a costed provision map and/or a Bristol Support Plan
  • email

If your pupil does not meet our criteria for direct input, email or contact your BAT Link for information about other support being given to pupils in your school who have met the criteria.
